Diabetes Self-Management Education Program

For:

  • Newly diagnosed patients
  • Uncontrolled Type 1 or Type 2 diabetics who have never had a formal self-management class

Program features:

  • An individual assessment prior to beginning program
  • Three classes utilizing ADA guidelines for the curriculum
  • Afternoon and evening classes available to meet your needs. We even offer a “Diabetes in a Day” Class!

We also offer:

  • Gestational diabetes classes
  • Diabetes support group

The Longstreet Clinic Diabetes Education Program is provided by a team of a Certified Diabetes Nurse Educator and a Registered Dietitian utilizing ADA guidelines for the curriculum. A physician referral may be required.

Dr. Andy Jaffal, Dr. Philip Marler, Dr. Lajos Toth, all Board Certified Internists practicing at The Longstreet Clinic, are credentialed by the ADA through the Diabetes Physician Recognition Program.
